How to request a "No Obligation" Proposal:

  1. Select the Proposal Adobe Acrobat PDF File from the links below.
  2. Print the form with your printer then complete, sign and date the form.
  3. Gather the following  information so AMCI can analyze your request.
    1. Current Accounts Payable Aging.
    2. Current Accounts Receivable Aging.
    3. Most Current Interim Company Financial Statement including a Balance  Sheet and Profit & Loss Statement.
    4. Most current year end Financial Statement including a Balance  Sheet and Profit & Loss Statement or the last filed company IRS Tax Return.
  4. Fax the completed form and necessary information to 1-866-400-2624.
