- Select either the Adobe Acrobat PDF File or the MS Word Doc from the
links below.
- Print the form with your printer then complete, sign and date the
form.
- Gather the following necessary information so AMCI can analyze your
request.
- Current Accounts Payable Aging.
- Most Current Interim Company Financial Statement.
- Current Accounts Receivable Aging.
- Year-end financial statement (or IRS Tax Return).
- Fax the completed form and necessary information to us at
